ADRS Consumer Satisfaction Survey for Persons Who are Deaf or Hard of Hearing

Alabama Department of Rehabilitation Services (ADRS) asked our team at Troy University to do an independent survey asking consumers like yourself about your experiences with vocational rehabilitation services.

Your name was selected because you received VR services from ADRS and your case was closed. We will add your responses to others who are being contacted.

This survey is voluntary. You may stop answering at any time. You may also decide not to answer any specific questions. All information collected will be kept private and stored in locked files.

By completing the survey, you are agreeing to participate in this study. The survey will take about 15-20 minutes of your time.
